ProView Credentialing Application
CAQH is an online database that collects information for the credentialing process. Completing the CAQH online credentialing application is a vital step of the onboarding process. This application is time sensitive and must be completed at least 8 weeks prior to your start date. Please begin this process immediately.
Note: Delay in completing your CAQH application may result in a delay of your start date. You do not need to have your licenses to start working on your CAQH application.
Application Instructions
Before you start the CAQH application you will need your CAQH Provider ID Number.

New Applicants
- Go to https://proview.caqh.org/pr
- Click “Register.”
- Enter your CAQH Provider ID Number.
- Enter your authentication data (e.g., SSN, DOB, etc.).
- Create Username and Password.
- Choose and answer three security questions.
- Acknowledge the Terms of Service.
- Complete the application and review data.
- Authorize Kaiser Foundation Mid-Atlantic to access your information.
- Verify your data entry – attest.
- Submit supporting documents.

Returning Applicants
- Go to https://proview.caqh.org/pr
- Enter your current CAQH Username and Password.
- Follow the prompts to create a new Username and Password.
- Choose and answer three security questions.
- Acknowledge the Terms of Service.
- Click “Update Account.”
- Confirm the CAQH Provider ID we have on file for you is correct.
- Proceed to the CAQH ProView home page.
- Complete the application and review data.
- Verify your data entry – attest.
- Submit supporting documents.
Frequently Asked Questions
For Primary Practice Location, please select the state that you will be practicing in. If you are already on CAQH, please change the Primary Practice Location to MD, DC or VA (whichever center applies) because if you list your current state (ex: GA or MA) they have different applications.
For your primary, practice please put down the Kaiser location where you will be working. You may click “import” then select your center from the drop down list; the information will be automatically populated. However, if your current practice uses the CAQH website, then do not change your primary practice location to reflect Kaiser, until closer to your final days with them.
If this is the case, then please put down Kaiser as your primary practice, and your current place of employment in the practice history section.

In order for CAQH to not hold up your credentialing profile, please say that you do not have a license. If you write in “pending”, they will hold your file until you get that information. Remember your CAQH app needs to be completed at least 6 week before your start date. You have to have a State license in order to begin the process (you can put either “Pending or N/A” for the license number and then put the License issuing state as either MD, DC or VA and then make up an expiration date (ex: 01/01/2012); you will be able to modify the document once your license is issued)

No, if you are not credentialed with the hospitals, please say that you are not privileged at any, otherwise this may hold up your CAQH app from being complete.

If you are insured by your current employer, please put down that information, you will then be required to fax in that current professional liability face sheet. If you do not have malpractice insurance (i.e., if you are coming out of training), then select the option for “not insured”. Do not select Import
If you have a CDS and/or a Federal DEA, please provide the license number, state the license is registered in and the expiration date. If you do not have a CDS and/or a Federal DEA, please answer the question “No”…Do not write the word pending because CAQH will require you to fax the license in and they will hold up your application.
