Health Screening Requirements

The Employee Health Department of Kaiser Permanente is happy to welcome you as a new provider to our organization. We are required by law to obtain certain information and documentation from you as newly employed healthcare providers.

It is imperative that your health screening is completed before attending New Employee Orientation. If you have any questions about the MyHealth portal, please refer to the Medical Center contact link below for your designated Employee Health Nurse.

At least two weeks prior to your start date, please follow the instructions here “MyHealth” Portal Instructions to complete your required Health Assessments.

You will need to call the Medical Record Number dept at 619-881-9954 between 8am-6pm, Monday-Friday to obtain your Medical Record Number (MRN) prior to setting up your ReadySet “MyHealth” portal and beginning your Health Assessment Surveys. You must tell the Kaiser Permanente representative that you are a New KP Employee.

NOTE: If you already have a KP MRN number (even if you are an inactive KP member) you will use your existing number.

INFLUENZA

The annual flu vaccine is mandatory as a condition of MAPMG employment.

To request an exemption, you will need to request an exemption form from your Employee Health Nurse, have it completed by a licensed allergist and email it to MAPMG EmployeeHealth@kp.org

To request an exemption due to a strongly held religious belief, you will need to request an exemption form from your Employee Health Nurse and email the completed form to MAPMG EmployeeHealth@kp.org.
