Health Screening Requirements
The Employee Health Department of Kaiser Permanente is happy to welcome you as a new provider to our organization. We are required by law to obtain certain information and documentation from you as newly employed healthcare providers.
It is imperative that your health screening is completed before attending New Employee Orientation. If you have any questions about the MyHealth portal, please refer to the Medical Center contact link below for your designated Employee Health Nurse.

You will need to call the Medical Record Number dept at 619-881-9954 between 8am-6pm, Monday-Friday to obtain your Medical Record Number (MRN) prior to setting up your ReadySet “MyHealth” portal and beginning your Health Assessment Surveys. You must tell the Kaiser Permanente representative that you are a New KP Employee
NOTE: If you already have a KP MRN number (even if you are an inactive KP member) you will use your existing number.
Annual influenza and COVID-19 vaccinations, including vaccinated with the 2023-2024 COVID-19 vaccine , are mandatory for all MAPMG physicians.

The flu vaccine is mandatory as a condition of MAPMG employment.
What if I have a medical contraindication to the flu vaccination?
To request an exemption, you will need to request an exemption form from your Employee Health Nurse, have it completed by a licensed allergist and email it to MAPMG EmployeeHealth@kp.org
What if I want to request a religious exemption from the flu vaccination?
To request an exemption due to a strongly held religious belief, you will need to request an exemption form from your Employee Health Nurse and email the completed form to MAPMG EmployeeHealth@kp.org.
COVID-19
The COVID vaccine is mandatory as a condition of MAPMG employment. Documentation is required for the initial series. If you did not receive the initial series or documentation is not available, you will need to receive the most current COVID vaccine.
What if I have a medical contraindication to the COVID-19 vaccination?
In rare cases, an allergy history to other vaccinations may present a contraindication to the COVID-19 vaccine. To request an exemption, you will need to request an exemption form from your Employee Health Nurse, have it completed by a licensed allergist and email it to MAPMG EmployeeHealth@kp.org

EXEMPTIONS: Please note that the exemption process for medical and religious exemption requests is interactive and can take some time to adjudicate. We encourage candidates to submit requests well in advance of anticipated start dates. If an exemption is approved, the exempted employee may be required to wear a mask and face shield (or take other steps as required by MAPMG) when at Kaiser Permanente facilities (including regional offices) at MAPMG's discretion, including during times of higher community transmission periods..
