CAQH Frequently Asked Questions

  1. The initial question asks "in which state does your primary office reside?" Should I put my current practice, or the Kaiser location where I will be working?

    For Primary Practice Location, please select the state that you will be practicing in. If you are already on CAQH, please change the Primary Practice Location to MD, DC or VA (whichever center applies) because if you list your current state (ex: GA or MA) they have different applications.

  2. The application asks for my Primary Practice Location, what should I put?

    For your primary, practice please put down the Kaiser location where you will be working. You may click "import" then select your center from the drop down list; the information will be automatically populated. However, if your current practice uses the CAQH website, then do not change your primary practice location to reflect Kaiser, until closer to your final days with them.

  3. What if my current practice does not use the CAQH website for credentialing purposes?

    If this is the case, then please put down Kaiser as your primary practice, and your current place of employment in the practice history section.

  4. Who is my credentialing contact?

  5. What should I put when it asks for a Medicare or Medicaid ID?

    This is not a required field and can be left blank, but if you have this information feel free to put it in the application.

  6. The application asks for my state license number, what if it's pending and I don't have this information yet?

    In order for CAQH to not hold up your credentialing profile, please say that you do not have a license. If you write in "pending", they will hold your file until you get that information. Remember your CAQH app needs to be completed at least 6 week before your start date. You have to have a State license in order to begin the process (you can put either "Pending or N/A" for the license number and then put the License issuing state as either MD, DC or VA and then make up an expiration date (ex: 01/01/2012); you will be able to modify the document once your license is issued)

  7. I know I will be floating between different offices, for primary practice location which address should I put?

    Please contact your recruiter to determine which location you will primarily be based out of.

  8. The CAQH asks me if I have any hospital privileges. I have just started applying to the hospitals listed on my contract, should I list this as "pending" on the application?

    No, if you are not credentialed with the hospitals, please say that you are not privileged at any, otherwise this may hold up your CAQH app from being complete.

  9. The application asks for TaxID #, Office Manager, Billing Contact, Insurance information, mid-level practitioners, interpreters, etc, how do I get this information?

    You will see an "import" button on the application, please click that. This information will be automatically populated for you. Anything that is not auto populated can be left blank.

  10. The application asks for a clinic schedule, office hours, times that patients are accepted, if I accept new patients, Medicare, Medicaid, etc. How do I answer these?

  11. Who should I put down as my malpractice/professional liability carrier, what is the policy number, do I have limited coverage, etc?

    If you are insured by your current employer, please put down that information, you will then be required to fax in that current professional liability face sheet. If you do not have malpractice insurance (i.e., if you are coming out of training), then select the option for "not insured". Do not select Import

  12. The application asks whether or not I have a Controlled Substance License or a Federal DEA, what if it's pending and I don't have this information yet?

    If you have a CDS and/or a Federal DEA, please provide the license number, state the license is registered in and the expiration date. If you do not have a CDS and/or a Federal DEA, please answer the question "No"...Do not write the word pending because CAQH will require you to fax the license in and they will hold up your application
